Whole Site Orientation for Family Planning and AYSRH
Whole site orientation (WSO) is a cost-effective approach to orient all health workers and staff – clinical and non-clinical at the health facilities – on family planning (FP) and adolescents and youth sexual and reproductive health (AYSRH). The orientation also includes components on post-pregnancy family planning (PPFP) and gender transformative programming using a low dose high-frequency mode. Each WSO session is about 1-2 hours and they are held over a period of two to three weeks.
WSO ensures knowledgeable staff become champions for FP and AYSRH and can directly counsel, provide services and refer for services appropriately.
Why Is Whole Site Orientation Important?
- Ensures that all facility/healthcare workers/staff (clinical and non-clinical), including CHVs/CHWs/VHTs, have accurate knowledge about FP/AYSRH and can share this knowledge and information with clients regardless of their age and parity
- Reduces missed opportunities for offering contraceptive services
- Empowers providers to offer provider-initiated FP services
- Sensitizes healthcare and other facility staff on gender-related issues that can hamper delivery of FP/AYSRH services
- Facilitates the integration of FP/AYSRH into other service delivery points
- Enables a critical mass of providers (clinical and non-clinical) to be reached with FP/AYSRH information without interfering with the routine facility schedule and in a cost efficient manner
- Builds a team spirit among facility staff through shared learning experiences and development goal
Evidence
WSO is a unique opportunity to strengthen the capacity of healthcare providers while reducing missed opportunities for staff to engage with potential FP clients. Since TCI’s inception in September 2017, over 13,168 staff from over 1,209 health facilities have taken part in WSO session and been oriented on FP and AYSRH services, according to program data as of December 2022. Intervention in Action: After Conducting Whole Site Orientation Sessions
“We have changed the way we offer our services. The capacity of health workers to provide quality family planning services has improved. Right now, everyone is confident to talk about family planning methods and counsel or advise clients on their method of choice. The moment you enter the health facility, everyone–starting with the askaris [security guards], cleaners, records persons to the health workers–can talk about family planning.
Due to our family planning signage, people are now more interested in asking about family planning and any person they approach readily provides them with reliable information. When they ask at the gate, the askaris directs them.
Family planning services have been integrated in all service delivery points. At the moment, all the care points at this facility are able to provide short-term family planning methods, while clients for long-term methods are referred to the family planning clinic.” — Alex Namaala Lwasa, nursing officer in Mukono, Uganda
Guidance: How to Conduct WSO
A.) Planning
- Identify knowledgeable providers on FP from the facility health management team who should be from the facility or from nearby facilities. These providers take the lead in conducting the WSO sessions.
- Once identified, orient and coach the identified staff on how to conduct the WSO. Consider starting with the high-volume facilities and in the subsequent pools include low-volume facilities, which can be merged for quorum and include nearby private facility staff.
- In consultation with the facility in-charge, the lead facilitator will establish the number of staff that will participate to guide the facilitator on the number of sessions/cycles that the facility will Ideally, a class should have between 10 to 25 participants.
- Agree with the participants upon the most convenient time to conduct the sessions.
- The lead facilitator and the facility in-charge work together to prepare the WSO schedule.
- Plan and prepare training materials including posters, PowerPoint presentation, flip charts, stationary etc.
- Identify a site/room within the facilities where the sessions will be carried out.
B.) During whole site orientation sessions
- Climate setting and sharing objectives with participants.
- Avail stationary, writing materials and handouts, IEC materials, and FP commodity samples if available.
- Participants sign the whole site attendance register every time they attend a session.
- The facilitator takes the participants through the day’s session as per the schedule, ensuring that its between 1-2 hours.
- The facilitator should use simple language that can be understood by all and avoid use of jargons.
- The facilitator should employ adult learning principles, such as role plays, demonstrations and group discussion.
- The facilitator should allow time for question-and-answer sessions and capture feedback and testimonials.
- The facilitator should thank the participants and remind the participants of the next sessions, encouraging them to continue and complete all sessions for certification.
C.) Post-whole site orientation
- Review the participant attendance register/list to assess the completion rate.
- Conduct a review of the processes and determine areas that require supplementation.
- Organize a new cycle if the facility still has capacity gaps and staff that require the orientation.
- After completion, the facility, in collaboration with the Health Management Team, should organize a whole site graduation where participants are recognized and certified.
Tips
- Maximum number of participants to be guided by level of facility and staffing patterns and other guidelines.
- Where TOTs are available at the facility level, make use of these as they are already familiar with the FP training curriculum and adult learning principles.
- Early morning, lunch time, afternoons and evening hours have been found to be ideal times to schedule WSO sessions.
- Where a local facility does not have the expertise in-house to facilitate a specific session, they can invite guest facilitators.
- PowerPoints can and should be shared with participants on WhatsApp or other digital platforms for future reference.
- After completion of the WSO schedule, identify participants that have completed 80% of the modules for graduation/certification and identify a potential pool for facilitating future sessions with new staff.
- Involvement of CHVs/CHWs/VHTs promotes community-based distribution of short-term methods, hence decongesting the health facilities.
Key Outcomes
- Percent increase in clients obtaining FP services

Monitoring Processes
- Track integration in other health service delivery points by the facility in-charge in collaboration with the FP nurse on a monthly basis.
- Support supervision on the quality of contraceptive service delivery and through the administration of the quality checklist on a quarterly basis and when the need arises.
- Obtain testimonials from the health facility staff through the Most Significant Change (MSC) technique.
- Provide ongoing monitoring of knowledge gaps during whole site orientation sessions through supervision on a monthly basis.
- Conduct client satisfaction exit interviews periodically.
Success Indicators
- Number of clients by gender receiving contraceptive services from other service delivery points.
- Number of staff by gender who have successfully completed the whole site orientation.
Cost
The cost elements include:
- Training materials, i.e., flipcharts, marker pens, notebooks, pens and copies of handouts (laptop and projector if available)
- Facilitation fee and CHMT/HMT supervision allowance
- Refreshments
- Printing of certificates
Sustainability
- Integrate and adapt the WSO sessions into regular facility updates, such as Continuous Medical Education opportunities
- Incorporate WSO sessions into facility work plans and local government work plans
